Decisions regarding whether, when, how, and what kinds of results from whole genome sequencing (WGS) should be disclosed to patients and families are fraught, involving a range of technical, clinical, ethical, and regulatory considerations. In particular, performing WGS under a research protocol but for a clinical purpose (e.g., to assist in the diagnosis or management of a serious health condition for which traditional approaches have been exhausted), places the interpretation of results and the decision about their return at the interface between research and clinical care. This case study will explore the decision making trajectory for the disclosure of whole genome sequencing (WGS) results to families of pediatric patients.
